Family Care Program - Established first in 1998 as a pilot
and later in 2007 as a regular entitlement program by the State
of Wisconsin, Family Care is a comprehensive, innovative
program combining service authorization and service provision
into one single setting with the purpose of helping individuals
maintain living independently and improve quality of life. It is
designed to give members better choices about what kinds of
services and supports they prefer to meet their individual needs.
The Project Access, Inc. Family Care Program is a contracted
Care Management Unit (CMU) for the Milwaukee County Department of Family Care. With a multidisciplinary team of professionals, the program works with seniors and adults who have both physical disabilities as well as mental health issues.
